Our approach
How we work
What TMS, Spravato and IV ketamine can realistically do for you, what side effects to expect, and what to watch for. We'd rather you decide with all the information than agree to something we haven't fully explained.

Who we help
Specialist care for patients medication didn't fully reach.
Optimum is built around one focused question: what comes next when standard treatment hasn't worked? You may be a fit if any of these describe you.

Our promise
Three things we won't do.
The clearest way to describe our approach is to name what it isn't.
A promise, not a pitch.
We won't oversell what these treatments can do. If TMS, Spravato, or IV ketamine isn't right for you, we'll tell you that - and if no interventional treatment fits, we'll refer you somewhere that does.
A plan, not a protocol.
Your treatment plan is built around your history, what you've already tried, and your goals - not a standard protocol applied to everyone who walks in. Multiple modalities means we can match the tool to your case.
A talk, not a contract.
The initial consultation is a conversation, not a commitment. You won't be asked to decide on the spot. You'll leave with a written estimate, a clear treatment recommendation, and time to think it over.
What happens
From first call to final session.
The journey is the same whether you end up on TMS, Spravato, IV ketamine, or a combination and we'll always be honest about which one fits, or if none of them do.
You reach out.
Call 614-933-4200 or send a message through the contact form. A team member will book your initial consultation and answer any quick questions about treatments, scheduling, or insurance before you commit.
You sit down with Dr. Blair or a Psychiatric NP.
You'll review your diagnosis, the medications you've tried, what worked and what didn't, and what you're hoping for. The consultation runs about 60 minutes. Bring any prior treatment records that would help.
The patient is scheduled for consultation
If a treatment fits, you'll see what the course looks like — number of sessions, schedule, what to expect, and the evidence behind it. We verify your insurance benefits in-house, submit prior authorization, and give you a written cost estimate before anything starts.
You start the course.
Sessions are about 20 minutes (standard TMS), 5 days for accelerated TMS, or as scheduled for Spravato and IV ketamine. No sedation, no anesthesia, no recovery time. Drive yourself, return to work, get on with your day.
You meet with Dr. Blair to review outcomes.
On your last treatment day, Dr. Blair sits down with you to review how the course went, what changed, and what the next step looks like - whether that's a maintenance plan, a referral back to your prescribing provider, or nothing further needed.
Pricing and questions
What it costs, and what comes up most.
Pricing is published. Insurance is verified before treatment begins. Below the pricing summary, the five questions we hear most often.
Pricing you'll see before treatment starts.
The initial consultation is a flat $200 - covered by self-pay or applied against treatment if you proceed. Self-pay pricing is published upfront for every treatment, and CareCredit financing is available subject to approval.
Frequently asked
The questions patients ask most.
Will my insurance cover treatment?
We're in-network with most major commercial plans, Medicare, Medicaid, Marketplace, and military/VA coverage - over thirty carriers in total. We verify your specific benefits before your first appointment, handle prior authorization, and provide a written estimate before treatment starts. See the full carrier list and pricing guide →
How do I get a written estimate?
Just ask. We provide a written estimate before any treatment begins — covering what your plan is expected to cover and your out-of-pocket cost. No surprises on the bill.
What does it cost to pay out of pocket?
Self-pay pricing is published upfront for every treatment. The initial consultation is a flat $200, and CareCredit is available subject to approval. No referral is needed to get started.
What does it cost to pay out of pocket?
Extremely rarely, yes — but the documented cases almost always involved undisclosed risk factors (personal or family seizure history, medications lowering seizure threshold, severe sleep deprivation). Modern protocols and proper screening make this exceptionally rare. We screen carefully for risk factors before every treatment course.
How many sessions does a course of treatment take?
It depends on the treatment. A standard depression course is about 36 sessions over roughly 7 weeks; OCD is about 29 sessions over about 6 weeks; and our accelerated option compresses treatment into 5 days. We'll map out your exact schedule and cost at your consultation.
Is maintenance treatment covered by insurance?
Maintenance TMS generally isn't covered by insurance. If it becomes part of your plan, we'll quote the cost upfront - same as everything else, no surprises.
Do I need someone to drive me home?
No. TMS doesn't sedate you, doesn't affect your thinking, and doesn't require recovery time. You can drive yourself to and from each appointment, return to work, and go about your day normally.
